One fateful evening, we received a distressing call from a client residing in Kololo, Kampala. The caller, a proud owner of Minitry, a beloved 7-year-old female German shepherd dog. He was deeply concerned about her sudden behaviour change. This ordeal began 2 weeks after a visitor, owner of a 3-month-old rescued puppy, paid a visit. By chance, the visitor left the puppy behind for the day.
The following morning, the visitor promptly retrieved the puppy, seemingly without incident. After some days, Ministy’s demeanour began to shift. Today, Ministy’s owners noticed troubling signs of lethargy, as she spent unusually long periods sleeping in the sun and isolating herself from her usual activities. As the day progressed, her condition deteriorated further, with weakness and increased isolation becoming more pronounced. Realizing the situation’s urgency, Ministy’s concerned owners reached out to us that fateful evening. Recognizing the gravity of the situation, we promptly scheduled an appointment for Ministy. Our veterinary team examined her and was eager to provide her with the care and attention she urgently needed.

PATIENT EXAMINATION
The medical team meticulously reviewed Ministry’s medical history and thoroughly examined her to uncover the root cause of her abnormal behavior. Despite our best efforts, the results of the initial examination proved inconclusive. To delve deeper into her condition, the medical team carefully collected samples from Ministry and subjected them to a battery of diagnostic tests. Despite these efforts, the test results yielded no definitive answers still. Undeterred, we integrated Ministy’s medical history, examination findings, and inconclusive test results to formulate a tentative diagnosis. This comprehensive approach allowed us to piece together crucial information and develop a working understanding of Ministy’s condition.
DIAGNOSIS
The medical team tentatively diagnosed Ministy with Ehrlichiosis, a concerning tick-borne illness known to affect dogs. Due to limited resources for confirming this disease in Uganda. We proceeded with treatment while maintaining a vigilant watch over her progress. The medical team promptly started Ministy on a course of medication tailored to combat Ehrlichia. Over the course of three days, we closely monitored her response to treatment. Ensuring that she received the care and attention necessary to combat the suspected infection. Despite the challenges posed by the lack of confirmatory testing, our priority remained squarely on Ministy’s well-being. Every effort was made to provide her with the best possible care under the circumstances.
OVERVIEW OF EHRLICHIOSIS
The bacterium Ehrlichia canis causes Ehrlichiosis, a tick-borne fever. The brown dog tick (Rhipicephalus sanguineus), endemic to Uganda, primarily transmits this infectious agent. These ticks become carriers of the bacteria as they feed on infected hosts. They transmit the disease during their blood meals at their various life stages. Furthermore, Ehrlichia can also be spread through transfusion of infected white blood cells to healthy hosts.
Symptoms of ehrlichiosis in dogs include fever, lethargy, shifting leg lameness, nosebleeds, among others. While it’s not typically considered a primary zoonosis, human infections have been reported. This highlights the importance of awareness and preventive measures. Prevention of ehrlichiosis involves rigorous tick control measures, utilizing medications and products proven effective against the brown dog tick. Any dogs slated for blood transfusions should undergo thorough screening to prevent the transmission of Ehrlichia or other blood-borne pathogens. By implementing these preventive strategies, we can mitigate the risk of Ehrlichia infection in both dogs and humans, promoting the health and well-being of all involved.
PROGNOSIS
After just two days of treatment, Ministy’s condition began to show remarkable improvement. By the third day, she had bounced back to her usual playful and affectionate self. This was much to the relief of her owners and our veterinary team alike. Recognizing the importance of completing the prescribed course of treatment, Ministy continued on her medication for an additional two days as recommended. Witnessing Ministy’s remarkable recovery was truly heartening. This serves as a testament to the effectiveness of prompt intervention and proper medical care. Her ehrlichiosis ordeal emphasizes the need for tick vigilance and the risks of brief encounters with infected ticks.
The suspected point source of Ministy’s infection was traced back to a tick likely acquired from the visiting puppy. This serves as a sobering reminder of the critical role ticks play as vectors for diseases such as ehrlichiosis.
